| 内容記述 | 要旨 : 心電図U波は，T波に引き続く小さな波形で，成因もまだ解明されていない．従来，U波が陰転する陰性U波は，虚血性心疾患の診断に有用であるとの報告があるが，詳細な検討がなされていなっかった．そこで，虚血性心疾患における陰性U波および陽性U波（異常陽性U波）の臨床的意義について，①責任冠動脈狭窄枝の推定が可能か？②左前下行枝の近位部狭窄と遠位部狭窄の鑑別が可能か？③左室後壁の虚血を示す，右冠動脈狭窄と左回旋枝狭窄の鑑別が可能か？④経皮的冠動脈形成術（PCI）の治療効果判定に有用か？の4点について検討をした結果，陰性U波および陽性U波が臨床的に極めて有用であった． The U wave is a small wave of unknown origin which follows the T wave on the electrocardiogram(ECG) . The negative U wave has been reported to be useful in diagnosis of ischemic heart disease but there has not been a detailed investigation. We investigated the clinical significance of the negative U wave and the abnormal positive U wave in ischemic heart disease, specificallywhether U waves can be used to: 1) estimate the location of stenosis of the coronary artery branches, 2)discriminate between proximal and distal stenosis of the anterior interventricular artery, 3) discriminate between right coronary artery stenosis and left circumflex artery stenosis that indicate the presence of posterior wall ischemia of the left ventricle, and 4) determine the outcome of percutaneous coronary intervention (PCI). We found that both the negative U wave and the abnormal positive U wave were clinically extremely useful. |
