| 内容記述 | 【要旨】 本研究の目的は, 看護師長が臨床実践・看護管理において体験した倫理的問題を調査し, 病院規模による違いを明らかにすることである. 看護師長が体験した倫理的問題と組織内の倫理に関する活動に関してA県下の全病院に勤務する看護師長1,938名を対象に自記式質問紙による調査を実施した. 病院規模を病床数300床で中小規模と大規模の2群に分け, 看護師長が体験した倫理的問題の頻度を比較した. 回答者524名中, 506名を分析対象とした（有効回答率26.1%）. その結果, 臨床実践の問題では, 病院規模に関わらず, ケア提供に関連する看護師不足, 抑制・鎮静等の共通の問題があったが, 中小規模病院の看護師長は非倫理的・能力が低い等の同僚と働く問題を, 大規模病院の看護師長は移植や未成年者, 重度障害の治療等に関する問題を有意に高い頻度で体験していた. 看護管理の問題では, 人的資源の不足は共通していたが, 大規模病院の看護師長はサービス残業や職員のルール違反等を中小規模の看護師長より有意に高い頻度で体験していた. しかしながらそれ以外の項目に病院規模にようる差は認められなかった. 【Abstract】 The purpose of this survey was to clarify differences in ethical issues of nursing practice/management as experienced by nurse managers based on the hospital size. An anonymous, self-administrated questionnaire regarding ethical issues in nursing practice/management and activities related to the ethics of organization was give to 1,938 nurse managers in the hospitals of A prefecture. Data were compared between two groups based on hospital size, hospitals with less than 300 beds (small-and medium-sized hospitals) and others (large hospitals). There were 524 respondents and 506 (a return of rate of 26.1%) were used for analysis. Regardless of the hospital size, a frequently encountered issue was shortage of nurses, which was related to provision of care, and problems of with factors, such as suppression and sedation. Nurse managers in small-and medium-sized hospitals experienced issues related to working with unethical/incompetent colleague sigunificantly more often than those in large hospitals. Nurse managers in large hospitals experienced issues related to related to transplantaitions, treatment of minors, severe failure significantly more often than those in small-and midium-sized hospitals. In nursing management, regardless of the hospital size, a frequently encountered issue was a lack of human resources. Nurse managers in large hospitals experienced issues related to overtime without pay and violation of rules significantly more often than those in small and medium-sized hospitals; in addition, other paremeters were not observed as a differences based on hospital size. |
