| 内容記述 | 心臓超音波検査における左室長軸断面像の描出は，第3肋間または第4肋間からのアプローチが適切とされているが，どちらの位置が適切か判断の基準は明確ではない．そこで若年成人における最適な走査位置を明らかにすることを目的として，肋間の位置の違いによる心臓の描出画像の違いを検討した．健常な大学生を対象に，第2肋間から第4肋間における左室長軸断面像をそれぞれ記録し，各肋間からアプローチした描出画像の違いについて検証した．適切な画像が得られたのは第3肋間で60％，第4肋間で40％だった．第4肋間が最適な走査位置だった群の特徴として男性が多く，身長と体重が高値の傾向が確認された．若年健常人でも第4肋間から良好な画像を得られた例が4割確認されたことから，必ずしも若年者で高位肋間からのアプローチが望ましいとは限らないことが示唆された．本研究の結果から，統計学的有意差には至らなかったものの，性別・身長・体重が肋間の位置決定に影響を与えた可能性が示唆された．今後はサンプル数を増やし幅広い年齢層を対象に，年齢や性別，体格を踏まえた更なる検討が必要であると考えられる． The purpose of this study was to clarify the most favorable intercostal space for scanning ultrasound images with the left ventricle parasternal long axis view in cardiac sonography among healthy young adults. This study obtained the approval of the Ethical Committee of Junshin Gakuen University. Signed informed consent was obtained from all participants prior to study. Ten healthy university students participated in this study. Four examiners recorded ultrasound images of participants from the intercostal spaces of the second, third, and fourth positions. The results showed that the most favorable imaging windows were as follows; third intercostal space, 60%; fourth intercostal space, 40％. Seventy-five percent of the “fourth intercostal” group were male participants and they tended to have greater values of height and weight than participants categorized in the “third intercostal” group. This study suggested that the imaging window of intercostal position to approach the parasternal long axis view among young healthy adults might not depend on age; however, determining the most favorable intercostal position could be influenced by factors such as sex, height, and weight, although the results of comparison did not reach statistical significance. Studies are needed that include a larger sample and wider age range for further investigation. |
