| 内容記述 | 要旨 : 米国ではPatient AdvocacyやPatient Representative等と呼ばれる専門家たちが, 患者と医療者双方にとって十分納得いく医療決断へと向かえるようそのプロセスを支援し, 結果として医事紛争の未然防止に一役買っている。患者は医療の主体であるという前提において患者は医療チームの一員であり, その認識のもとで, 第三者である彼らも医療チームの重要な一員なのである。近年, 我が国においても患者アドボカシーという呼称でその役割が紹介され, 医療決断サポーター(支援員)等, 日本版患者アドボカシー人材育成プログラムが諸所で実施されてきたが, 人材の具体的な運用について広く理解を得られているとは言い難い。本稿は, まず医療安全管理の観点から, 事後対応によっては患者の満足が必ずしも得られないことを医事関係訴訟と裁判外紛争解決の現状分析に基づき述べた上で患者アドボカシーの役割と意義を論じ, 次に現実的な運用方法の提案へと項を進めたものである。更に, この分野においては実証的研究がほとんどなされていない現実を踏まえ, 第三者の立会いを患者はどう捉えるかを問うたアンケート調査結果に基づき考察を深めた。 Abstract : In the United States, a third party called “Patient Advocacy” intervenes in medical care and supports clear medical decisions for both patients and health care providers as a member of the medical team. In addition, with respect to a medical safety management, medical disputes can be avoided by this type of intervention.In recent years, some Patient Advocacy training programs have been carried out in our country since the role of Patient Advocacy in the United States was introduced. However, there still seems to be too many difficulties for the professionally trained personnel to work in a concrete way.First, in this article, a role and the significance of Patient Advocacy were confirmed by the comparison with the present states of the medical lawsuits and the alternative dispute resolutions. Furthermore, the author discussed the modality of the intervention by a third party in medical care in our country, based on the results of the questionnaires that have been obtained from outpatients. |
